Who May Be Eligible for MIH Services

MIH services may benefit individuals who need additional healthcare coordination, support after a hospital stay, or assistance accessing ongoing community resources and care. Services are designed to help reduce preventable hospital visits and support safer transitions from hospital to home.

Frequent Emergency Care Use

Support for individuals with frequent 911 or emergency department visits.

Recent Hospital Admission

Support for individuals transitioning home following hospitalization or acute care treatment.

Chronic Health Conditions

Including heart failure, COPD/asthma, diabetes, pneumonia, and other qualifying conditions.

Healthcare Provider Referral

Patients referred by healthcare providers for additional support services.
